🟣 CCC Plus – Virginia
CCC Plus is a managed long-term services and supports program for Medicaid members who are 65+, disabled, or medically complex. It includes interdisciplinary care coordination and access to home- and community-based services.

🔵 DD Waivers – Virginia
Virginia offers three Developmental Disability waivers: Building Independence, Family & Individual Support, and Community Living. These waivers support individuals with functional, behavioral, or medical needs across home and community settings.

🟢 CAP/C – North Carolina
CAP/C supports medically fragile children (ages 0–20) with home- and community-based services like assistive technology, nursing care, and respite. Families may direct their own care through consumer-directed options.
