CD stands for “Consumer Directed.” It’s a care option where you hire and manage someone you trust to help with daily tasks.

You choose! Many select a family member or close friend. The person must be 18+ and pass a background check.

We walk you through it! Start with a short intake call. Then we guide both you and your chosen aide through orientation.

Virginia Medicaid uses Managed Care Organizations (MCOs) to provide services under CCC⁺ and DD Waivers. All plans offer basic benefits — and some include extras like dental, vision, or transportation.
Choosing a supported plan ensures smooth coordination and access to the care you deserve.

🗂️ 1. Verify UAI Status / Verificar estado UAI
We request a copy of your Uniform Assessment Instrument (UAI) to confirm waiver eligibility. We can get it from DSS, Health Dept., or your current agency. Solicitamos una copia de su Instrumento de Evaluación Uniforme (UAI) para confirmar la elegibilidad para la exención.
🩺 2. Medicaid Profile Review / Revisión de perfil de Medicaid
We check your Medicaid status for DD or CD services, then schedule your intake. Verificamos su estado de Medicaid para servicios DD o CD y luego programamos su evaluación.
🧩 3. Build Your Plan of Care / Diseñe su plan de atención
Together, we create a support plan—Personal Care, Respite, or Companion Care if eligible. Creamos juntos un plan de apoyo: Cuidado Personal, Respiro o Compañía si califica.
📤 4. Submit for Authorization / Enviar para autorización

Who makes the request? You or your caregiver must contact your local Department of Social Services (DSS).

  • 📧 Share your contact info so they can schedule the visit
  • 👩‍⚕️ A nurse will reach out within 1–2 weeks to begin the UAI

📋 Developmental Disability (DD) Waiver Pathway

If you believe you're eligible for the DD Waiver, you must contact your county’s Community Services Board (CSB) or Behavioral Health Authority.

  • 📞 Ask to begin the process for DD Waiver eligibility
  • 🧠 A formal assessment will be completed by your local CSB
  • ✅ If approved, you’ll be placed on the statewide DD Waiver Waitlist
  • ⏳ Wait times vary — slots open gradually based on priority status

💡 Good news: While waiting for DD Waiver availability, many individuals qualify for Personal Care Services through the CCC+ Waiver. We can help with that process now.
