Schedule a UAI Assessment
You can request an LTSS screening through your local Department of Social Services. A Community-Based Screening Team (i.e.: Social Worker and Health Dept. Nurse), will meet with you and a family member/caregiver, if available, to assess your need for the CCC Plus waiver. If you are hospitalized and need a screening, this can be done in the hospital by a discharge planner; You must be eligible for Medicaid to receive and have waiver services paid by Medicaid.
