Agency-Directed vs. Consumer-Directed
Personal care and respite services can be provided through either an agency-directed model, where a personal care agency manages the services, or a consumer-directed model, where the individual manages their own care.
Service Authorization
The Department of Medical Assistance Services (DMAS) or its service authorization contractor (Insurance Company) must authorize all waiver services.
Managed Care
For individuals in the Cardinal Care managed care program, the Managed Care Organization (MCO) provides service authorization functions.

Eligibility

Individuals 65 or older, and those with disabilities of any age, may be eligible.
Services
The waiver covers a variety of services to enable individuals to live at home, such as personal care assistance, homemaker services, and transportation.
Screening
A screening process is required to determine eligibility and identify the specific services needed, using a tool called the Uniform Assessment Instrument (UAI).
Your local Department of Social Services (DSS) and the local health department conduct the screenings for the CCC Plus Waiver.
